| Overview |
|
| It is the responsibility of the social medical services division to integrate
social work with overall hospital care and provide a broad range of services to
the elderly, disabled, families, and individuals who need assistance in
maintaining or achieving their full potential for self-direction, self-reliance
and independent living. |

| Mission |
|
|
|
Social Medical Services division provides medical assistance and services
without discrimination of any kind such as race, age, color, religion, sex,
disability, handicap (including AIDS and AIDS-Related conditions), political
belief or national origin.
Social Services respond to written and verbal referrals from any source (i. e.,
physician, hospital staff, patients, families, community).
The referral should indicate the nature of one problem. Social Services will
assess the condition and provide the appropriate intervention.

| Vision |
|
|
The psychosocial needs of the individual and available resources shall determine
whether social services can be provided.
A summary of observations will be noted in the medical record of any ongoing
contact with the patient and family. The intervention will be entered on the
Progress Notes or the Case Management Progress Notes in the patient’s medical
chart
The objectives of the Social Medical Division are as follows:

To define the department’s role as an integral part of the hospital’s provision
of health care.

To ensure that the provision of health care services to the patient and their
family promotes the psycho-social well being of the hospital patient.

To provide guidelines for the patients, family, employees in order to gain
their psychological and social trust.

| Responsibilities |
|
-
To provide appropriate care tailored to the patient’s psycho-social need to
meet the psychosocial needs through assessments and referral, counseling,
crisis intervention, and the establishment and facilitation of support groups
for patients/families, and team members.

To contribute to a treatment plan responsive to patient/family concerns.

To offer appropriate resources to assist families in their financial
obligations by exploring and activating potential assistance.

To coordinate administrative and clerical activities within the department.
-
To direct coverage to the ICU and Emergency Medicine Department (EMD).
-
To assess and intervene high risk esp. for follow-up services.
-
To assess and refer of suspected abuse and neglect cases
-
To coordinate newborn adoption.

To coordinate medication supplies with pharmacy department.
-
To provide services such as rehab/nursing home placement, transportation,
medicines, shelter, equipment, counseling, etc.

To provide social services supportive services to the employees of the
hospital.

To coordinate with non-profit organizations, foundations and committees.

To participate in hospital committees/meetings, community activities and
provide lectures and seminars to community groups
